New Patients

Welcome! We are Dr. James Nass and Dr. Calen Leider of River's Edge Family Dental, a general dental practice in Plymouth, Wisconsin.

As patients ourselves, we understand the idea of visiting the dentist makes many people anxious, therefore we believe being prepared and knowing what to expect at your first appointment can help to alleviate your concerns. 

At the first appointment, we will gather information about your health history and any current oral health concerns. We will do a complete oral examination, including evaluation of the bone structure around each tooth and determine what kind of cleaning is best suited to you. If necessary, we may take x-rays to help evaluate the health of each of your teeth. The exam will conclude with an oral cancer screening. Finally, we will develop a treatment plan, taking the time to explain in detail any recommendations we make and answer any questions you may have. 

If you would like to expedite the check-in process, we are happy to have patients complete the required forms and bring them along to the first appointment. You will find links to the forms below.
